MINNEAPOLIS-ST.PAUL, MN — Minnesota Department of Health announces that there will be fun summer prizes for those who get the COVID-19 vaccination between May 27 to June 30, 2021.

They aim to reach their 70% goal of Minnesotans aged 16 and older to be vaccinated.

If you are a Minnesotan, this is your chance to have a safe and enjoyable summer! Between May 27 and June 30, 2021, the first 100,000 Minnesotans aged 12 and above who receive their first COVID-19 vaccine will be eligible for a special prize.

From June 1 to June 30, the awards request form will be available on this site. Requests for rewards must be submitted to the Minnesota Department of Health by June 30, 2021, at 11:59 p.m. The Department of Health will conduct a verification of vaccine information.

We’re on track to accomplish our target of vaccinating 70% of Minnesotans aged 16 and up by July 1. You can now contribute. So, getting your vaccination, protecting yourself, and starting off summer the correct way are all rewarded in ‘Your Shot to Summer!’

The summer rewards are all listed below.

1. Great Lakes Aquarium Pass

One free admission to the Great Lakes Aquarium in Duluth. This offer is valid until July 1, 2023. The Minnesota Department of Health will provide the Aquarium with recipients’ contact details, which will be used to mail tickets to Minnesota residents who choose this option.

2. Mall of America Nickelodeon Universe Pass

Qualified for a 30-point Nickelodeon Universe ride pass which can be used until September 1, 2021. The Minnesota Department of Health will provide recipients’ contact information to the Mall of America, which will deliver instructions on how to claim the pass.

3. Minnesota Fishing License

Through February 2022, you are qualified for one individual Minnesota resident yearly fishing license. A Minnesota fishing license is required for the recipient. The Minnesota Department of Health will provide winners’ contact information to the Minnesota Department of Natural Resources, which will notify Minnesota residents to finish their fishing license applications.

4. Minnesota State Parks Vehicle Permit

Qualified for a one-year vehicle permit from Minnesota State Parks. The Department of Natural Resources will send the permit to Minnesota residents’ email. The Minnesota Department of Health will give the Department of Natural Resources the recipients’ contact details, which will be used to mail the State Parks vehicle permit.

5. Minnesota Zoo Admission

Through September 8, one adult entrance to the Minnesota Zoo is free. The Minnesota Department of Health will supply the Minnesota Zoo with beneficiaries’ contact details, which will be used to provide information to Minnesotans so they can redeem their ticket.

6. Northwoods League Baseball Tickets

One reserved ticket to a Northwoods League baseball game during the 2021 season is provided. The Minnesota Department of Health will give the Northwoods League with recipients’ contact details, and Minnesotans will phone the team’s ticket office they want to book their ticket, providing their full name and address for verification. Tickets are available on a first-come, first-served basis. The Rochester Honkers, Willmar Stingers, Mankato MoonDogs, St. Cloud Rox, and Duluth Huskies are among the teams competing.

7. State Fair Tickets

Two entry tickets to the Minnesota State Fair in 2021 are available. The Minnesota Department of Health will supply the State Fair with winners’ contact information, and tickets will be emailed no later than July 16, 2021.

8. Valleyfair Single-Day Admission

During the 2021 season, it’s valid for one Valleyfair entry ticket and the opportunity to buy additional tickets on the same date at a discounted price. Minnesotans will receive a unique code through email from the Minnesota Department of Health to redeem this offer. In the 2021 season, Valleyfair will be closed on Mondays and Tuesdays. Before choosing this as your vaccination prize, we recommend that you check valleyfair.com for the most up-to-date operating dates and hours.

9. Visa Card

You’ll be given a $25 Visa Card that you may use anywhere Visa is accepted. Minnesotans will get the cards in the mail from a vendor contracted by the state of Minnesota. Visa cards are not available for delivery to PO Boxes.

From June 1 to June 30, these rewards will be given to the first 100,000 Minnesotans who complete their first shot information and make a request. Requests for rewards must be submitted to the Minnesota Department of Health by June 30, 2021, at 11:59 p.m. The Department of Health will conduct a verification of vaccine information.

Minnesotans can also sign up for a prize by calling (833) 431-2053 from 9 a.m. to 7 p.m. Monday through Friday and 10 a.m. to 6 p.m. on Saturday.

To be qualified, Minnesotans aged 12 and up must get their first injection between May 27, 2021, and June 30, 2021.

The Minnesota Department of Health is gathering information from you so that you can apply for a reward through the COVID-19 vaccine reward program, named “Your Shot to Summer.” To be eligible for a reward, you must fill out the form with your personal data, including if and when you took your first dose of COVID-19 vaccine. Under the Minnesota Government Data Practices Act, your personal information is considered private data.

This information is collected to confirm your qualification for a reward through this program and to deliver the reward that you requested, as supplies permit. Employees and contractors of the Minnesota Department of Health with a need to know, as well as other persons or entities as authorized by law, will have access to your personal information.

If you choose a reward that is being given by another organization, that organization will receive your name, postal address, or email address so that they can mail you the reward or the details you need to access or redeem the award.

In order to complete the form, you must be at least 18 years old at the time of vaccination. Please have your parent or guardian fill the form if you are aged between 12 and 17. The form for an unaccompanied minor can be filled out by someone acting as a parent or guardian.

Please verify your contact details and mailing address before submitting the form to ensure receipt of your prize. You may use any address where you can receive mail if you do not have a permanent postal address.

Regarding Visa cards, you will receive a confirmation email within 24 hours of submitting your reward request. Please resubmit your rewards request if you do not receive a confirmation email – or if you do receive a confirmation email, but your mailing address is not correct.
