The claim form is ridiculously easy to fill out. Just follow along and provide you name, address, phone number, email address (that you use with your Facebook account at the time of April 22, 2010 and September 26, 2011.

Then you choose your preferred method of payment!

There are a handful of options Venmo and Pay Pal are among them.
